This report seeks to gain a better understanding of police-involved deaths, and to provide specific recommendations to address how to prevent these incidents from arising, and how to best address investigations into these deaths. Furthermore, it sets out a model of investigation and oversight that ensures a sufficient degree of accountability will
be obtained.
